Participation in SWYBB includes acknowledgment of risk and authorization for emergency medical care if needed.
Medical Release & Liability Waiver
I am aware that with participation in sports comes the risk of injury, and I understand that the degree of danger and seriousness of risk varies significantly from one sport to another. I acknowledge and accept the risks inherent in athletics in which my child will be participating.
In the event of illness or accident, I give permission for emergency treatment by qualified medical personnel for my child. I give consent for the facility to secure any and all necessary emergency medical care for my child.
I agree to hold the South Western Youth Basketball Organization (SWYBB), its Board, Coaches, and any volunteers harmless from any and all liability, actions, claims, or demands of any kind and nature whatsoever that may arise by or in connection with my participation in SWYBB events.
